Do not use GCC __sync spinlocks if GCC version is less than 4.1. This 
fixes a compilation problem on Ubuntu 6.06 LTS.

Our 'Passenger' namespace may conflict with application models that happen 
to be named 'Passenger' as well. Fix this.

Do not use apr_bucket_pipe anymore, because it only closes the pipe if 
reading succeeded. Replace it with our own bucket implementation that 
never closes the pipe. This fixes http://tinyurl.com/6b8nku

Refactoring: move spawn server idle time information to AbstractServer. 
This makes it easier in the future to have dynamic, configurable spawn
