Always log exceptions/errors to the log file #92

[reported by blelbach] [Trac time Wed Aug 17 14:53:55 2011] Whenever an exception/error occurs, it should always be reported to a log file (preferably a local one to ensure that it is not lost). The HPX version info (AGAS version, Boost version, compiler version, HPX revision) should also be reported.
