Leaks into other files are detected and avoided! Previously we could leak into other files if they were use/required/done in the same lexical scope as autodie. Fixed nasty bug where autodie/Fatal could use filehandles from the wrong pkg. This was caused because we were caching on the short name of subroutines (eg, 'open') rather than their full name ('main::open').
