Major overhaul of the avatar chooser.

I think I'd like to do a bit more work on this, since it's very tightly coupled to organizations, users, and Sentry Apps (the later being the more egregious problem)

For now here's my refactor that at least makes using it feel better

Though I think there are some regressions wrt saving states. But I think I can fix that in a follow up.

Will add screenshots later
