Use PhutilClassMapQuery instead of PhutilSymbolLoader

Summary: Use `PhutilClassMaQuery` instead of `PhutilSymbolLoader`, mostly for consistency. Depends on D13588.

Test Plan: Poked around a bunch of pages.
