VSCode Extension: Listen For File Changes

Currently if we use the vscode p5-studio extension then the webview in vscode does not refresh if index.js is changed. So we have to listen the file for any changes and refresh the webview.

To provide the best user experience we will be removing the webview and port it to native browser app with live server. We will be using the webview for inbuilt docs and I will open a seperate issue about that. We are removing this as VSCode webview is limited to not fancy things.
