I was hoping you'd have something sane for a list of plugins.
Well.
Any one of those could be a problem, particularly anything that reacts to no-permission players, or enforces a region system, or anything like that...

LOL, I was expecting that response. It is absolutely insane.

I did some more testing (aka spawning citizens in every single world) and found out that it's the WorldGuard flag entry: deny. Is the only solution for this to remove the flag?

Ah yup that flag would do it :P
You'd have to complexify in some manner:

  • Beg worldguard to support NPCs better
  • Make the flag not apply to the default group, but move all actual players out of default.
  • ...
