This makes some display issues at least on Linux and Windows 8.1 when switching to window mode.

Set window title to game name once when known
Remove window title setting from UI, as game name is not available at
this point, instead use the generic application name first.
Before this change, the window title was set to an empty string, causing
the window manager to name the window 'Unknown Window' in the task lists
and such. For fullscreen windows, this would last until close.
For windows not in fullscreen state, this was 'fixed' by UpdateTitle()
after a second.

This one got mostly fixed by #792, the rest is WontFix (→ #179 - SDL2 specific bug) for now.
Maybe when we have another UI besides SDL someday this will not be an issue anymore.
IMO we could close this and reopen #179 instead.
