Add SIM card model

The provider requires the following data from users buying a SIM card:

• First name
• Last name
• Date of birth
• Sex
• Full address of accommodation (eg. hotel name, street, zip code, city, hotel phone)
• E-mail address

For internal use, we need also:

• SIM Card number
• Activation state (If we do not order in one batch...)
