Report the remote port number in MON$ATTACHMENTS [CORE5028]

Block progress on DNET652

I have mentioned this missing column from monitoring table which can be useful for port examination from monitoring tables and Dmitry Yemanov confirmed me this to be easy and maybe possible to add into next Release Candidate of 3.0.

Implemented as a part of CORE2493. MON$REMOTE_ADDRESS now contains the <IP>/<port> string.
