Toggle Pairwise Subject type: algorithmic | peristent #1303

There are two ways to generate a pairwise id: either algorithmically, or persistently.

Algorithmically sends the subject identifier as a hash fo the client_id and inum (or sector identifier uri).

Persistent creates an LDAP attribute called ppid for the user, and stores an entry for each client.

Somehow, we lost the ability to set whether pairwise ids should be algorithmic or persistent. I'm sure this feature existed in the past.
