Refactor loader.js to allow zero-downtime restarts #2012

Part 3 of 3 of the "hotreload" feature branch

  • Re-factor loader.js so that it is not so noob (really)
  • Re-factor app.js so that it signals parent when it is ready to listen
  • loader.js updates to sequentially kill old child process and replace it with new child process
