Mac OS X midi code style issues #216

Just the minor code style changes mentioned commit 4cc8afe. Creating an issue or we might forget.

Normally I compile with:
-Wmissing-prototypes -Wstrict-prototypes -Wold-style-definition

Fixed a few that I indicated via a comment in the PR (which is not viewable in the commit .. lolz).

with theses includes :+1: #include <AudioUnit/AudioUnit.h>
#include <AudioToolbox/AudioToolbox.h>
#include "types.h"
#include "midi.h"

I have theses errors :
[SRC] Compiling audio/midi_osx.c
In file included from /Users/dversdivers/code/OpenDUNE/src/audio/midi_osx.c:6:
/code/OpenDUNE/include/types.h:103: error: two or more data types in declaration specifiers
/code/OpenDUNE/include/types.h:103: warning: useless type name in empty declaration

(OS X 10.4.11 with latest Xcode available for it)
