Revised pre-release

Revised Proof Version

This release takes into account a number of comments on release 0.1, and also incorporates a newly drafted "quick start" section (at pp.109ff). Subject to final proof-reading and to some further work on the index, it is essentially in final form, and should be usable.

First pre-release

This release is complete and indexed, but is effectively an alpha release. I believe that this is usable, but it is probably not in final form.
