Sometimes a localized %SIG isn't restored properly. Consider:

----8<----
local $SIG{__WARN__} = sub { warn "outer: $_[0]" };
warn "one\n";
{
  local $SIG{__WARN__} = sub { print "inner 1: $_[0]" };
  warn "two\n";
  {
  local %SIG;
  warn "three\n";
  local $SIG{__WARN__} = sub { warn "inner 2: $_[0]" };
  warn "four\n";
  }
  warn "five\n";
}
warn "six\n";
__END__
outer: one
inner 1: two
inner 1: three
inner 2: four
five
outer: six
----8<----

Line 5 should be "inner 1: five". Likewise:

----8<----
local $SIG{__WARN__} = sub { warn "outer: $_[0]" };
warn "one\n";
{
  local $SIG{__WARN__} = sub { print "inner 1: $_[0]" };
  warn "two\n";
  {
  local %SIG;
  warn "three\n";
  $SIG{__WARN__} = sub { warn "inner 2: $_[0]" };
  warn "four\n";
  }
  warn "five\n";
}
warn "six\n";
__END__
outer: one
inner 1: two
inner 1: three
inner 2: four
inner 2: five
outer: six
----8<----

This happens only if the complete %SIG is localized.

From @jimc

restating what OP reported,

bug in 1st example is fixed if either -
local %SIG before warn "three" is removed
local keyword on
$SIG{__warn__} = sub{}.
before warn "four" is DE-localized ("local" removed)
