If you press Ctrl+C in a terminal that was used to launch qubes-manager, an error dialog pops up complaining about a keyboardinterrupt, but qubes-manager does not quit. However, it then no longer notices that a VM has been started. It's then necessary to quit and restart qubes-manager.
