Continuing on the topic of problems with VMs that have passthrough devices: some other time the driver (this time tests for iwlagn) loads fine, but the interface is not created (not seen by ifconfig -a). The only error message is in xl dmesg:

mm.c:907:d16 Error getting mfn 100000 (pfn 942b1) from L1 entry 8000000100000673 for l1e_owner=16, pg_owner=32753

Seems like this was related to mfn fragmentation, a problem that seems to be gone with xen 4.1.1-3.
