FWIW, installing KDE (using sddm) and anti-evil-maid seems to fix it.

Qubes OS version:
R4.0-rc4
Steps to reproduce the behavior:
Assign external monitor as primary display in the laptop BIOS settings; Then boot Qubes.
Expected behavior:
As with R3.2, both grub UI and the Linux parts of the boot process are displayed on the external monitor. User can see the disk decryption prompt as well as the desktop login prompt.
Actual behavior:
Grub menu is shown on external display, but the desk decryption prompt and login prompt are both invisible if the laptops internal display is not in line of sight. User must type both passwords blindly (actually pretty difficult for Xfce login dialog).
Once desktop login has been achieved, system acts as expected with the internal display turned off and the external display used as primary.
General notes:
This relates to a common use case where the user docks or otherwise stows their laptop when at their desk, and the internal screen is not physically visible.
