UEFI Installer Crashes #3995

Qubes OS version:

R4.0

Affected component(s):

Anaconda, Xen, Qubes OS

Steps to reproduce the behavior:

Open boot menu and select UEFI USB.

Expected behavior:

The USB boots successfully to Anaconda and the GTK installer starts.

Actual behavior:

Xen boots via the EFI Loader, then the text disappears and there is a black screen afterwards.

General notes:

Have tried both USB and CD/DVD to no avail.
