Closing until somebody reproduces it. Also note we just upgraded to fc20, which might just fixed that anyway.

Hello,
Actually I do reproduce the trouble (on Qubes R2 and fc20 template).
(I'm not the original poster).
I've got a big uptime (23 days), and this morning, pulseaudio decide to restart itself.
All my already started VM cannot produce any more sound, with the exactly same error message in pacat.log
I don't know if it's important for this problem, but I've got serious trouble with pulseaudio and Qubes concerning the sound.
Frequently, the pulseaudio inside a VM just get "stuck", and in pacat I have "stream underrun" multiples times.
If you want that I do more test , I'm on Qubes-user and my e-mail is briaeros007 on gmail :)

Sound doesn't work in some VMs. The easiest way to spot it is grayed out "attach/detach audio-input" option in Qubes Manager. This isn't clear yet which VMs have this problem and why.
I've found one VM with this problem and its pacat log contains:
Log modification time is the same as VM startup time.
This is DispVM started from autostarted AppVM.
