Errors and warnings should indicate origin #282

Hi,

It would be much easier to debug problems if warnings and error messages would print the name of the file that caused them, as well as the line number.

Right now, messages of this nature are not very helpful:

[2014-11-26 21:42:13] [WARNING] Error: Annotation "parameters" is not allowed on comment from type "unknown"

This is a really great tool, and you all are super responsive. If you guys think of something on the less-technical side you could need help with, I'd be very happy to contribute some of my time.
