The patch redirects to ItemNotFoundUrl if the GET request to the Index action of FormBuilderController is executed with the fxb.FormItemId and fxb.HtmlPrefix parameters.

The release was tested with the following Sitecore products in bare condition (no hotfixes, patches or other customizations, unless explicitly specified):

  • Sitecore CMS 9.0 rev. 180604 (Update-2)


  • bin/Sitecore.Support.400109.dll
  • App_Config/Include/zzz/Sitecore.Support.400109.config


For all Sitecore instances in the solution:

  1. Make a backup of the files mentioned in Contents section and listed in the installation instruction (if any).
  2. Copy the contents of the Website folder from the archive to the Website folder of your Sitecore solution, overwrite existing files if any conflicts occur.
  3. Change the Controller field's value to Sitecore.Support.ExperienceForms.Mvc.Controllers.FormBuilderController, Sitecore.Support.400109 in the /sitecore/layout/Renderings/System/Forms/Mvc Form item.
  4. Publish the change.
  5. (Optional) Copy the archive into the $(data)/patches folder permanently.


  1. Remove the files listed in the Contents section.
  2. Restore the original files from the backup made during installation.
  3. Change the Controller field's value to Sitecore.ExperienceForms.Mvc.Controllers.FormBuilderController, Sitecore.ExperienceForms.Mvc in the /sitecore/layout/Renderings/System/Forms/Mvc Form item.
  4. Publish the change.
