I think it'd be a good thing to swap the documentation over to a wiki here on GH. That way, it can be collaboratively edited with forks/pulls just like the code itself. As it stands, the help menu is not particularly helpful or even functional.

The editor manual and Q3 documentation is hosted on the end user site. At least for the time being, I suggest pointing the general documentation to the URL below:

http://icculus.org/gtkradiant/documentation/

Some of the help files are game specific, so different gamepacks have different documentation. Those help files should still point to the local help files installed on the users system unless the game has better documentation elsewhere.

All the documentation is woefully out of date. A wiki for a complete rewrite isn't a bad idea, though even collectively it'll be a pretty big undertaking. I haven't used GitHub's wiki so not sure how well it works compared to other wiki packages out there.

Related, as mentioned by Mateos81: The following commands don't work:

Help > Manual F1 (does nothing)
Help > Bug report (directs to the defunct http://qeradiant.com domain)
File > Check for Radiant Update (option is disabled)

Additionally, if you set up GtkRadiant for both Quake 3 and Quake Live, there are two documentation flyouts even though they are exactly the same docs.
