Updated design and style for 2020. #21

Things changed:

  • Added setup instructions to readme.
  • Changed icon on page to new blue outline. Favicon unchanged.
  • Updated to new Darkly theme using Bootstrap 4. jQuery and knockout updated as well.
  • Fixed background image dragging around when clicking student toggle.
  • Added 馃帀 and reformatted signup complete page.
  • Changed placeholder text to actual placeholder values.
  • Defaults to student options visible.
  • Changed student checkbox to Yes/No buttons to match rest of form.
  • Better invalid student number text.
  • Updated to use new logo.
  • Everything left-aligned, with heading and pay buttons full width.
  • Heading adapts depending on screen size.

Looks great, some much needed changes.

I have made some suggestions for corrections/further improvements.
