system.local always returns 0 token #1467

[#1467]: Fix token returned by system.local
Summary:
While populating system.local we fill entries with filtered tablet server info.
But index is not filled correctly, because it incremented after filtering.

Fixed by moving index increment to the start of the loop.

Also fixed host_id provided by system.local.

Test Plan:
Launch local cluster, use cqlsh to connect to all nodes one by one.
Check that tokens reported by system.local and system.peers are not duplicated.
