Once we have a default stylesheet (#76), the hardcoded CSS in the HTML5 backend should be removed.

We could consider leaving it in under the condition for which it serves, which is to patch the default asciidoc.css to match our markup. But when the default Asciidoctor stylesheet (theme) is specified, or a custom stylesheet, it definitely doesn't belong there.
