<p>Description:</b><br>
Delivers individualized palliative care to clients at their home/institution utilizing a
team of nursing and home support staff specially trained in theoretical and practical
palliative care skills. This program training includes: communication, pain symptom
control, personal care, dying process, grief process and stress management.</p>
