<p align="center"><a name="Pre-Operative Instructions"><font size="4"><strong>Pre-Operative
Instructions</strong></font></a></p>
<p><strong>1.</strong>&nbsp;&nbsp; Do not eat, drink, eat candy, chew gum or smoke the day
of surgery. (This means from midnight on).&nbsp;You may brush your teeth but do not take
sips of water.</p>
<p><strong>2.</strong>&nbsp;&nbsp; Have a shower or bath and wash your hair the day before
or the morning of your surgery.</p>
<p><strong>3.</strong>&nbsp;&nbsp; Remove any make-up and nail polish prior to coming to
the hospital... this includes toe nail polish.&nbsp; </p>
<p><strong>4.</strong>&nbsp;&nbsp; Please leave all valuables at home... no jewellery
(rings, necklaces, earrings and anklets)... please do not bring money to the hospital the
day of surgery.&nbsp; DO NOT WEAR CONTACT LENSES.</p>
<p><strong>5.</strong>&nbsp;&nbsp; If staying overnight in the hospital, arrange for a
family member or friend to bring all luggage to the Pre-Surgery Clinic.&nbsp; Please bring
personal items such as toothpaste, toothbrush, soap, deodorant, tissues, etc.</p>
<p><strong>6.</strong>&nbsp;&nbsp; Wear comfortable clothing to hospital (example, track
suits).</p>
<p><strong>7.</strong>&nbsp;&nbsp;&nbsp; Confirm the time of surgery the day prior to
surgery by calling your surgeon's office.&nbsp; Arrive at the Pre-Surgery Clinic at the
appointed time for all of your appointments and for surgery.&nbsp; Bring your white
hospital card, your health card and any insurance information.&nbsp; Please reconfirm your
insurance coverage with your employer.&nbsp; Private and Semi-private coverage may not be
covered under your insurance plan for rehabilitation care.</p>
<p><strong>8.&nbsp;</strong>&nbsp;&nbsp; If being discharged the same day of surgery,
ensure that you are accompanied home and that someone stays with you overnight.&nbsp; Do
not drive for 24 hours.</p>
<p><strong>9.&nbsp;</strong>&nbsp;&nbsp; Discharge time is 0900 hrs for admitted
patients.&nbsp; Please arrange your ride home for that time.</p>
<p><strong>10.&nbsp;</strong> Relatives who have made arrangements to speak to the surgeon
after surgery should wait in the 3C Waiting Area.&nbsp; After speaking to the surgeon,
proceed to the appropriate nursing unit, eg. 1B, 1C, 3C, Surgical Day Care.</p>
<p><strong>11.&nbsp;</strong>&nbsp; If you have breathing problems, and take puffers,
please bring them with you.</p>

<p align="center"><a name="Paediatric Pre-Operative Instructions"><font size="4"><strong>Paediatric
Pre-Operative Instructions</strong></font></a></p>
<p><strong>1.&nbsp;</strong> Do not let your child eat, drink, eat candy or chew gum the
day of the surgery (this means from midnight on).&nbsp; Keep children away from water as
they may try to drink unless otherwise directed by physician.</p>
<p><strong>2.&nbsp;</strong> Have a shower/bath and wash child's hair the evening before
surgery.</p>
<p><strong>3.&nbsp;</strong> Remove any make-up and nail polish prior to coming to the
hospital.&nbsp; This includes toe nail polish.</p>
<p><strong>4.</strong>&nbsp; Please leave all valuables at home.&nbsp; No jewellery
(rings, necklaces, earrings and anklets).&nbsp; Children with long hair should have their
hair tied back with elastic bands... no metal.</p>
<p><strong>5.</strong>&nbsp; If staying overnight in the hospital, arrange for a family
member to bring all luggage and personal items to hospital after the surgery.&nbsp; Do not
bring luggage to the Pre-Surgery Clinic or 1C until after surgery.</p>
<p><strong>6.&nbsp;</strong> Wear comfortable clothing to the hospital.&nbsp; (example
Track suits).</p>
<p><strong>7.</strong>&nbsp; Confirm the time of surgery the day prior to surgery date by
calling your surgeon's office.&nbsp; Arrive at the Pre-Surgery Clinic at the appointed
time for all appointments and for surgery.&nbsp; Bring your child's white hospital card,
Health Card and any insurance information with you. </p>
<p><strong>8.</strong>&nbsp; Mother, father, or legal guardian should bring the child to
the hospital. &nbsp; A parent should stay in the hospital the entire day of surgery.&nbsp;
One parent is allowed to stay overnight if the child is being admitted.</p>
<p><strong>9.</strong>&nbsp; Children being discharged from hospital the same day of
surgery need a parent and another adult to take the child home (one person to drive and
one person to take care of&nbsp; the child).</p>
<p><strong>10.&nbsp;</strong> Discharge time is 0900 hours for admitted patients unless
otherwise directed. &nbsp; Please arrange a ride home for that time.</p>
<p><strong>11.</strong>&nbsp; Please wait in the 3C Waiting Area if you wish to apeak to
the surgeon following surgery.&nbsp; After speaking to the surgeon, please wait for the
Recovery Room staff to meet with you.</p>
<p><strong>12.&nbsp;</strong> If you have breathing problems and take puffers, please
bring them with you.</p>
