| <p>The number of outpatient visits to the Ambulatory Care Department in the 1998/99 fiscal | |
| year were approximately 42,000.</p> | |

| The Ambulatory Care Department provides multidisciplinary care in the following | |
| specialties:<br> | |
| Gynaecology clinics which include infertility, colposcopy and LEEP; Medicine clinics which | |
| include endocrinology, pain, epilepsy, rheumatology and recovery for procedures carried | |
| out in imaging such as biopsies; and surgical clinics which include plastics, vascular, | |
| general, fracture, breast and ophthalmology. The ophthalmology clinics include laser | |
| treatments, eye, ultrasound, fluoroscein angiography and visual field studies.</p> | |

| <p>The number of outpatient visits to the endoscopy suite in the 1998/99 fiscal year | |
| exceeded 4,700.</p> | |

| <p>The endoscopy suite provides a variety of diagnostic procedures; gastroscopies, | |
| duodenoscopies, colonoscopies, flexible sigmoidoscopies, ERCP's, bronchoscopies and other | |
| procedures to control haemorrhage in the gastro-intestinal tract as well as dilatation and | |
| haemorrhoid banding. The portability of the systems allows procedures to be done at | |
| the bedside for patients in the critical care areas of the hospital.</p> | |

| Providing therapy for musculoskeletal, multiple trauma and catastrophic injuries, the | |
| Rehabilitation Centre specializes in individualized, immediate access to professionals in | |
| a range of professional disciplines, specifically for Motor Vehicle Accident patients and | |
| for those with Insurance Coverage.</p> | |

| <p><strong>Musculoskeletal - Soft Tissue Injury Programme</strong><br> | |
| The Programme consists of physical therapy intervention with consultation from other | |
| disciplines as required.</p> | |
| <p><strong>PHASE I:</strong><br> | |
| Intervention and treatment incorporates education and pain relieving modalities | |
| (Electrical therapies, Ultrasound, Hydrotherapy, hot and cold therapies).<br> | |
| Initiation of early individualized functional movement exercises and mobilizations to | |
| improve range of motion and flexibility.</p> | |

| Progression of individualized active exercise programme to include strengthening in | |

| The Programme is an extension of the physical therapy intervention programme (as outlined | |

| <p><strong>Catastrophic - Acquired Brain Injury Programme</strong><br> | |
| The Programme provides a fully comprehensive multidisciplinary focus with the following | |

| Physical assessment of neuromuscular status with treatment focusing on maximizing the | |
| client's mobility and functional level.<br> | |
| Assessment of a client's cognitive abilities with treatment directed at re-establishing | |
| pre-trauma levels of functioning.<br> | |
| Functional capacity evaluation, work conditioning, job or home site analysis and | |
| management of the return to work process.<br> | |
| Short term psychotherapy to reduce anxiety and disability related to trauma.<br> | |
| Assessment, treatment and management of clients with deficits in speech, language or | |
| hearing in order to maximize communicative functioning.<br> | |
| Individual, marital and family counselling, and discharge planning to promote psychosocial | |
| adjustment and coping skills.</p> | |

| <p><a name="critical"><font color="#001058" size="5">Critical Care: ICU/CCU</font></a></p> | |
| <p><b>Facts:</b></p> | |
| <p><font face="default">In the 1998/99 fiscal year, 442 patients were admitted to ICU and | |
| 390 patients were admitted to CCU.</font></p> | |
| <p><b>Features:</b><br> | |
| The eight-bed Intensive Care Unit and eight-bed Coronary Care Unit support patients from | |
| surgical, cardiology, medical, oncology, renal and maternal child programmes. With access | |
| to state-of-the-art equipment, care is provided by a multidisciplinary team including | |
| physicians, nurses, nutritionists, pharmacists, respiratory therapists, physical and | |
| occupational therapists, social workers and chaplains.</p> | |
| <p>The Intensive Care Unit provides services that include cardiac monitoring, hemodynamic | |
| monitoring and ventilator support, continuous veno-venus hemodialysis and peritoneal | |
| dialysis. The Coronary Care Unit provides services that include cardiac monitoring, | |
| recovery from heart attacks and other cardiac supports, temporary transvenous pacemaker | |
| insertion, heart muscle biopsies, bundle of HIS studies and cardioversions. | |
| Education for both patients and their families is a large component of care in the | |
| critical care setting.</p> | |

| <p><a name="diabetes"><font color="#001058" size="5">Diabetes Education</font></a></p> | |
| <p><b>Facts:</b><br> | |
| The Diabetes Education Centre is open from 0800-1600 hours Monday to Friday and during the | |
| evening from 1830-2100 hours when the evening group program is in progress, (three | |
| evenings per month), patients are accepted with a written referral from a registered | |
| physician.<br> | |
| Programs for diabetes management include three half-day group education sessions, group | |
| evening programs, individual counseling and ongoing follow-up care. This past year the | |
| Diabetes Education Centre registered over 5,700 patient visits.</p> | |
| <p><b>Features:</b><br> | |
| The Diabetes Education Centre at The Credit Valley Hospital provides assessment, | |
| counseling and education to both inpatients and outpatients with diabetes mellitus (Type 1 | |
| and Type 2) and gestational diabetes.<br> | |
| The philosophy of the Diabetes Education Program focuses upon education, teaching, and | |
| information sharing. The philosophy encompasses the belief that improved patient | |
| understanding, increased patient awareness, health promotion, and professional input into | |
| patient problem solving can, and does lead to an improved quality of life for the patient | |
| with diabetes.</p> | |

| <p>The Diagnostic Imaging Department will perform over 100,000 examinations this year. | |

| <li>5,000 nuclear medicine exams</li> | |
| <li>8,500 C.T. exams</li> | |
| <li>5,500 mammography exams</li> | |

| <p>The Imaging Department at The Credit Valley Hospital is equipped with state-of-the-art | |
| technology which allows us to provide the best service possible to our community. Imaging | |
| services are provided on an inpatient, outpatient and emergency basis. Emergency coverage | |
| is offered 24 hours a day, seven days a week. The majority of imaging examinations are | |
| performed digitally which allows the information to be stored by a computer system. These | |
| images can be transferred electronically to many areas of the hospital, to your doctor's | |
| office or other medical institutions. </p> | |

| Provides optimal level of emergency care to all patients through immediate triage | |
| assessment prioritization and intervention. </p> | |
| <p>Provides Emergency Care 24 hours a day, seven days a week.</p> | |
| <p>Annual patient visits are 60,000. The Credit Valley Hospital Emergency Department | |
| is among the busiest "ER's" in Metro and 905/GTA.</p> | |
| <p><b>Features:</b></p> | |
| <p>Recent expansion of the Emergency Department was completed in 1998 to increase | |
| treatment spaces to 34 within three patient care zones.</p> | |
| <p>Care Centre One: Provides care for emergent, acutely ill patients, e.g. Trauma, | |
| Resuscitation, etc.</p> | |
| <p>Care Centre Two: Provides care for urgent priority patients requiring care and | |
| treatment.</p> | |
| <p>Care Centre Three: Provides a Clinic Environment for ambulatory patients | |
| presenting with non-urgent patients.</p> | |
| <p>Quick Response Program: QRP staff provide early intervention and discharge | |
| planning to assist patients and families to manage care at home with appropriate supports | |
| and community services.</p> | |
| <p>Crisis Intervention Team: The Crisis Intervention Team functions as an integral | |
| part of the emergency team for management of patients and families presenting with | |
| psychosocial problems.</p> | |
| <p>The type of care provided in the "ER" has shifted over the past several | |
| years, with more clinical intervention, diagnostic testing and observation care being | |
| provided in the "ER" in place of admission to a hospital bed whenever possible.</p> | |
| <p>Paediatric visits are one-third of the visits to "ER" or approximately 20,000 | |
| per year. The high volume of paediatric patients requires additional resources | |
| and specialized skills among the multidisciplinary team for children and families.</p> | |

| Skylight Meal Services Operating a "Front Door Barbecue" cart during the summer | |
| months. Permanent Pizza & Pasta Bar, and Bakery & Coffee Cafe. Serving 1,200 | |
| customers daily with 600 at lunch service. Nutrition Assessment and Education Ontario | |
| College of Dietitians' Registered Dietitians provide nutritional expertise for renal, | |
| oncology, paediatrics, general medicine clients. Patient meal choices by computerization | |
| patient meal selections are computer-driven. To accommodate individual food preferences | |
| and dislikes, each patient/client is contacted within 24 hours of admission by a food | |
| service representative or a volunteer.</p> | |

| More than two million lines of medical information is transcribed each year. A full | |
| range of centralized transcription services are available, including: history and | |
| physical examination reports, consultations, operative reports, pathology reports, x-ray | |
| reports, etc.</p> | |
| <p>Approximately 300,000 copies of patient-related information are sent out each year. | |
| Millions of pages are scanned into CVH's optical system, to be merged with the | |
| electronically created information for viewing in the on-line patient record.</p> | |

| The Health Information Management Department performs the following functions regarding | |

| <li>maintains an incomplete record management system to assist physicians in ensuring | |

| <li>codes all records accordingly to the ICD-9-CM classification system</li> | |
| <li>abstracts relevant data from patient records, for submission to the national reporting | |
| agency (CIHI)</li> | |
| <li>provides statistical analysis services</li> | |
| <li>handles requests for release of information</li> | |
| <li>provides copies of patient information to authorized recipients, for purposes of ongoing | |

| <p>Recognized leaders in the electronic record progress, Health Information Management at | |
| CVH is among the first hospitals to institute hospital-wide electronic signature for all | |
| transcribed reports. They have two work-from-home projects underway: medical | |
| transcription and coding. Within the next few months, they will extend their coding | |
| to include Emergency Room information.</p> | |

| Each patient is assigned a primary nurse as they join the program. The nurse will assess | |
| and plan the care program with the patient and family. Each patient completes a customized | |
| in-hospital education programme before performing their own home dialysis. The | |
| program currently has 95 patients.</p> | |
| <p><b>Features:</b><br> | |
| Provides a program of nursing care, education and support for patients with end stage | |
| renal disease who wish to perform their own dialysis at home.</p> | |

| <p><a name="inpatient"><font color="#001058" size="5">In-Patient Pharmacy</font></a></p> | |
| <p><b>Features:</b><br> | |
| Point of use and automated tablet counter technology utilized as part of the drug | |
| distribution system. Comprehensive intravenous admixture service including chemotherapy | |
| and total parental nutrition. Drug Use Evaluation programme ensures cost-effective and | |
| quality pharmaceutical services. Pharmacists are an integral part of the multidisciplinary | |
| team and provide patients with pharmaceutical care. Consolidated pharmaceutical | |
| procurement has been introduced to achieve effective inventory management. Fully | |
| computerized pharmacy system which is integrated with many other hospital modules.</p> | |

| <p><a name="Infection Prevention & Control"><font color="#001058" size="5">Infection | |
| Prevention & Control</font></a></p> | |
| <p><strong><font color="#001058">Facts:</font></strong></p> | |
| <p><font color="#001058">The Infection Prevention & Control Unit is managed by the | |
| infection control officer, and laboratory program manager. Infection control | |
| practitioners are responsible for ground level investigation and control, and work in | |
| close harmony with the Microbiology Laboratory. An infectious diseases specialist is | |
| also available for consultation.</font></p> | |
| <p><strong><font color="#001058">Features:</font></strong></p> | |
| <p><font color="#001058">The Infection Prevention & Control Unit's mandate is the | |
| prevention and control of nosocomial (hospital-acquired) infection in patients, staff and | |
| visitors. This is achieved through continuous surveillance, epidemiological | |
| investigation of clusters of infection, education, consultation, policy and procedure | |
| development in liaison with hospital programmes and research.</font></p> | |
| <p><font color="#001058">Patients in-hospital, who are found to have infections of certain | |
| types may be isolated in various ways, and visitors and family may be requested to assist | |
| by paying special attention to handwashing. These time honored | |
| "containment" techniques for infections are simple but powerful, and are | |
| increasingly important as all health care facilities face the challenge of organisms that | |
| have become resistant to many antibiotics.</font></p> | |
| <p><font color="#001058">The CVH Infection Prevention & Control Unit would like to | |
| remind you to get your Influenza A (flu) shot if you fall into one of the following risk | |
| groups, or have close contact i.e. family member, with someone who falls into one of the | |
| following risk groups. (The flu shot is given <u>every year in the autumn</u>.):</font><ul> | |
| <li><font color="#001058">persons age 65 or older</font></li> | |
| <li><font color="#001058">anyone with chronic heart, lung or kidney disease</font></li> | |
| <li><font color="#001058">anyone who lives or works in a nursing home</font></li> | |
| <li><font color="#001058">anyone with diabetes, cancer, a blood disorder or an immune | |
| problem</font></li> | |
| <li><font color="#001058">children and teenagers (ages 6 months to 18 years) who have been | |
| treated with acetylsalicylic acid (aspirin) for long periods</font></li> | |
| </ul> | |
| <p><font color="#001058">Persons who fall into the above high-risk categories can go to | |
| their family physician for the flu shot, free of charge. All others, including | |
| contacts may purchase the vaccine at their local pharmacy. </font></p> | |
| <p><font color="#001058">As well, persons who fall into the following risk groups below | |
| who have not had the pneumococcal vaccine can get this shot at the same time as the | |
| Influenza vaccine:</font><ul> | |

| <p>The Department of Laboratory Medicine at the Credit Valley Hospital serves both an | |
| inpatient and outpatient population, performing over two million tests annually. The state | |
| of the art laboratory provides all services to assist the physician in diagnosing | |

| <p><b><u>TRANSFUSION MEDICINE</u></b> </p> | |
| <p>The role of transfusion medicine is to provide compatible blood and blood products for | |
| medical and surgical patients as well as to perform prenatal and post-partum blood | |
| grouping antibody screening for obstetrical patients. </p> | |
| <p><b><u>HAEMATOLOGY</u></b> </p> | |
| <p>The role of haematology is to test for changes in blood cells in diseases such as | |
| anaemia infection and leukemia. This includes examinations of peripheral blood and bone | |
| marrow. In addition, this department tests the blood clotting mechanism. </p> | |
| <p><b><u>HISTOLOGY/CYTOLOGY</u></b> </p> | |
| <p>The histology/cytology department has the responsibility for the preparation of all | |
| tissue specimens and body fluids removed at surgery, by needle biopsy or aspiration. The | |
| prepared tissue is examined by a Pathologist, who then makes a diagnosis. </p> | |
| <p><b><u>MICROBIOLOGY</u></b> </p> | |
| <p>The Microbiology Department is responsible for the isolation, identification and | |
| subsequent antimicrobial susceptibility testing of bacteria, fungi, and pathogenic | |

| <p><b><u>CLINICAL CHEMISTRY</u></b> </p> | |
| <p>The chemistry department involves testing in the areas of routine chemical | |
| constituents, enzymes, lipids, protein, protein electrophoresis, therapeutic drug | |

| <p><b><u>GENETICS</u></b> </p> | |
| <p>This division includes clinical and laboratory services. Families are referred | |

| <p>In 1998/99, the psychiatric team provided inpatient mental health services to enable | |
| the acutely ill patient to achieve optimum health, wellness and integration back into the | |

| <p>The role of the 2D multidisciplinary team is to deliver specialized programs to Peel | |
| residents with severe, acute mental illness or mental health problems. To support | |
| this role, emergency services, short-term inpatient assessments, stabilization, short-term | |
| treatment and discharge planning are core components of the 2D inpatient psychiatric | |
| program.</p> | |
| <p>Multidisciplinary care and treatment were provided for approximately 500 patients, with | |
| an average length of stay of 14.23 days per patient.</p> | |
| <p>The mental health population is particularly vulnerable because of the nature of their | |
| problems. Most experience significant difficulties achieving the basics in life, | |
| such as shelter and job, as well as appropriate treatment and community support. The | |

| life skills, focus, activity, relaxation and exercise) to facilitate improved patient | |
| functioning in activities of daily living, adult social, recreational and employment roles | |

| enable the patient to attain optimum health and independence. The rehabilitation | |
| multidisciplinary team collaborates closely with the community and health providers to | |
| facilitate the ongoing care of our patients after hospital discharge.</p> | |
| <p>In 1998/99, the rehabilitation team provided inpatient rehabilitation programs and | |
| services to enable the physically and cognitively challenged patient to achieve optimum | |
| health and independence. Multidisciplinary care and treatment was provided for 280 | |
| patients, with an average length of stay of 39 days per patient. 83% of the Rehab | |

| <p><b><i>Paediatrics:</i></b> The paediatric speech language pathologists in the | |
| department provide assessment, evaluation, treatment and management for preschool children | |
| with a variety of communication difficulties including problems with articulation and | |
| phonology, language, fluency, voice and resonance are seen up until the age of 5 years. | |
| (School-age children are serviced through their school boards).</p> | |
| <p><b>Facts: </b><br> | |
| The department provides services in a regular program, as well as in a team setting in | |
| Child and Family Services. All of our paediatric staff members work in conjunction | |
| with therapists from Peel Regional Preschool Speech and Language Services. This | |
| coordination of staff allows for children to be seen much sooner than in previous | |
| years.</p> | |
| <p><b>Features:</b><br> | |
| A new service delivery model was designed in mid-1998, when the Peel Regional Preschool | |
| Speech and Language Services program was implemented. The goal is to provide improved | |
| services for the previously under-serviced preschool population. Emphasis was placed | |
| on establishing a central referral base and waitlist, combined with coordination of speech | |
| language pathology services throughout the Peel Region. Intensive training | |
| workshops have been established for parents in order to teach family members how to | |
| encourage development of language skills, correct sound production, or fluent | |
| speech. A significant emphasis is placed on home programming. Direct therapy | |
| continues to be available to those children who require a more intensive approach, both in | |
| group formats as well as on an individual basis.</p> | |
| <p><b><i>Adult Services:</i></b> Adult services are provided on an inpatient and | |
| outpatient basis, for communication and swallowing disorders. A variety of communication | |
| disorders are treated, including dysarthria, dyspraxia, aphasia, voice and cognitive | |
| communication difficulties. Patients require a physician referral, and are prioritized | |
| according to the severity of the problem.</p> | |
| <p><b>Facts:</b><br> | |
| Approximately 80 - 90% of the inpatients referred to the speech language pathologists | |
| require intervention around swallowing, in addition to any communication difficulties they | |
| are experiencing.</p> | |
| <p><b>Features:</b><br> | |
| The department also provides services in several team settings, for patients with | |
| communication and swallowing disorders, on the acute care and rehabilitation units | |
| (in-patients) and in the rehabilitation day hospital (out-patients).</p> | |
| <p>The speech language pathologists use a variety of techniques when required for a | |
| complete assessment of a patient's swallowing, which may also involve the use of | |
| videoflouroscopic procedures.</p> | |
| <p>Patients requiring augmentative communication aids may be either directly serviced by | |
| one of our speech language pathologists (for low-tech devices), or the speech language | |
| pathologist may act as a facilitator in the assessment provided by external professionals | |
| (in the case of high-tech aids).</p> | |
| <h3>Audiology Services</h3> | |
| <p>The audiologists provide a wide range of services to a diverse age range of patients. | |
| There is no restriction on eligibility for service. Services include conventional, sound | |
| field or play audiometry, impedance testing (tympanometry, reflexes and reflex decay), | |
| auditory evoked potential testing for thresholds and neurological concerns. They | |
| also provide hearing aid evaluation for all ages, with selection and fitting of a complete | |
| range of amplification devices.</p> | |
| <p><b>Facts:</b><br> | |
| The audiologists see upwards of 2,800 patient visits per year, of which 60% of | |
| the patients are paediatric (under the age of 16).</p> | |

| Infants as young as 3 months of age can be assessed. The department also oversees | |
| the implementation of an Infant Hearing Screening Program which aids in the identification | |
| of hearing impairment at an early stage of development. Follow-up hearing aid | |
| re-assessments of the new hearing aids ensure the desired benefit has been obtained, along | |
| with subsequent monitoring of the hearing and the hearing aid fitting to ensure the | |
| desired benefit has been obtained, and continues, are also part of our audiology services.</p> | |

| <p><a name="spirit"><font color="#001058" size="5">Spiritual & Religious Services</font></a></p> | |
| <p align="left">Spiritual & Religious Services is committed to addressing the | |
| spiritual and religious need of patients, families and staff. We seek to help those | |
| at The Credit Valley Hospital use their own faith, faith symbols and practices to move | |
| them in a direction that is creative, wholesome and healthful. Thus, we endeavour to | |
| generate a warm, caring and trusting environment where relationships can grow.</p> | |
| <p align="left">The co-ordinator of the Spiritual and Religious Services facilitates | |
| meeting the spiritual and religious needs of all patients. On a regular basis, | |
| patient visitation, counseling and support for ethical deliberation is provided.</p> | |
| <p align="left">The secretary of Spiritual and Religious Services is available to meet and | |
| welcome all visitors to the department. She receives telephone calls and referrals | |
| requesting spiritual assistance, will gather information and contact the appropriate | |
| person. The secretary is available to assist you in locating a particular patient | |
| when necessary.</p> | |
| <p align="left">The Spiritual and Religious Services Advisory Committee serves as an | |
| advisory body to review, assess and evaluate pastoral care issues both through the | |
| resources of the Hospital and the religious community. The committee is made up of | |
| thirteen representatives from the faith community and from hospital staff.</p> | |
| <p align="left">The spiritual and religious needs of patients and their families may | |
| be met through patient visitation conducted by: the co-ordinator of Spiritual and | |
| Religious Services; visitors from the department, trained in dealing with visitation, | |
| grief and bereavement; the patient's own religious leader; and representatives from the | |
| patient's congregation. We will contact community faith groups for patients wishing | |
| to receive visits from their religious leaders and take referrals from staff who feel | |
| spiritual support is required for patients and their families.</p> | |
| <p align="left">The spiritual and religious needs of staff may be met through staff | |
| visitation conducted by the co-ordinator of Spiritual and Religious Services.</p> | |
| <p align="left">Weekly Christian interdenominational services are held in the Meditation | |
| Lounge on Sundays at 2:00 p.m. The Meditation Lounge, on Level 2 off the main lobby, | |
| is available to everyone as a quiet place for prayer and meditation. A Christian | |
| Communion service is celebrated on the third Wednesday of each month in Nursing Units 1D | |
| and 1E at 1:15 p.m. A Roman Catholic Mass is celebrated in the lounge on the first | |
| Thursday of each month at 2:00 p.m. A special memorial service is held every two | |
| months for patients who have died: families and staff are welcome to attend this | |
