<li> <p ALIGN="JUSTIFY">Ideal to start DTP, OPV, and HIB series at 2 months of age for full term and
prematures. Series may be started as early as 6 weeks of age. </p>
<br><li>
<p ALIGN="JUSTIFY">DTP: Diphtheria, Tetanus, Pertussis. Minimal interval between first three doses is 4 weeks. For booster dose, minimal interval is
6 months. DTaP (Diphtheria, Tetanus, and Acellular Pertussis) is licensed only for the 4th and 5th doses.</p>
<br><li>
<p ALIGN="JUSTIFY">Polio: Oral Polio Vaccine (OPV) or Inactivated Polio Vaccine (IPV). Primary series consists of 3 OPV. When medically indicated, IPV may be given so
long as a total of 3 doses of OPV and/or IPV are completed. First two doses
are given at least 6 weeks apart, with third done at least 6 months after the
second dose.</p>
<br><li>
<p ALIGN="JUSTIFY">HIB: Haemophilus influenza type B vaccine. Two types approved for use in
infants: PRAXIS-LEDERLE HBOC (HibTifer) and MERCK SHARP &amp; DOHME
PRP-OM P (PedvaxHIB). The two vaccines have different dose numbers and
schedules. The schedule above is that of the Praxis-Lederie product supplied
by the Public Health Division. Refer to your protocols for management of infants
with a mixed vaccine history. HIB vaccines may be administered as early as
6 weeks of age. When an infant may not return at 2 month intervals, vaccine
may be given 1 month after previous dose except for the booster dose which
should be given at least 2 months after the last dose. Children 15-59 months
of age should receive 1 dose. Children older than15 months of age requiring multiple
doses please refer to protocols or vaccine package insert.</p>
<br><li>
<p ALIGN="JUSTIFY">MMR Measles, Mumps, Rubella vaccine. Routine schedule is two doses of MMR, the first at 15 months of age a
second dose at 4-6 years of age Measles and rubella are required for school entry. During epidemics of measles or
when travelling to such areas, infants 6-11 months of age should receive monovalent measles antigen, followed by
MMR at 15 months of age and 4-6 years. Children 12-14 months of age in epidemic areas or going to such areas
should receive an MMR to be followed by a second MMR after 15 months of age may not be given. It is not necessary
to repeat MMR if child received single antigen measles for 4-6 year dose.</p>
<br><li>
<p ALIGN="JUSTIFY">MMR, DTP of DT, HIB and OPV can be given simultaneously at separate site to children 15 months of age or older.</p>
<br><li>
<p ALIGN="JUSTIFY">The fifth dose of DTP and fourth dose of Polio usually is given after the 4th birthday and is required for school entry.
This dose may not need to be given if the fourth DTP and third Polio were given after the 4th birthday.</p>
<br><li>
<p ALIGN="JUSTIFY">Td: Adult type Tetanus, Diphtheria vaccine. To be used after the 7<sup>th </sup>birthday.</p>
<br><li>
<p ALIGN="JUSTIFY">Measles and Rubella vaccines are required for school attendance through high school. Mumps vaccine is not required for school entry at any level.</p>
<br><li>
OPV is not required for anyone after the 18<sup>th</sup>birthday.
