I might have found the cause of my previous issue.
I just wrote a comment on Berekuk's play-perl post. After reading his post, I logged in, then went to the comment box. I hit preview, and when happy, I hit submit. I got the following:
Maybe this is what I got last time, but didn't notice as I moved on to the next thing. I went back in the browser, copied the text, went to the home page, clicked on the article again, pasted the content and did submit without review, and everything worked fine.
Just tried this again, doing a reply to a comment (on the same post). Review then submit result in the above error. A straight submit works fine.
I just had the exact same problem, on the exact same post. Wonder if it's specific to that particular post, for some reason ...
Update: nope, it's not only that post. Just got it again on a different post. :-/
I can confirm this behaviour. Just saw it on a post I tried to comment on - after I previewed the comment.
I'm also affected for more than a month, maybe two. User: dolmen.
I tried to disable Ghostery and to switch to a different browser (Opera vs Firefox) without more success.
What is the workaround? Avoid preview?
Preview, copy the text when you're ready. Logout, quit the browser, dance around naked, then login again.
After clicking Preview, always go Back to the post page before doing anything, regardless of whether you want to edit your comment further or post it as it is. Never submit the comment from the preview page, always from the post page. At least, this works 100% reliably for me.

Yes, the mt_session table has been truncated. It didn't fix the problem :-(

Still seems to be an issue - any chance of adding a note to the preview page to warn people about this?
