Replace Test::Output::Tie with Capture::Tiny #2

Using tied handles to capture output has several limitations that
Capture::Tiny overcomes. In particular, it does not break code
that expects standard output handles to appear open and it preserves
existing layers like :utf8.

Test::Output::Tie is deprecated, but left in the distribution
for backwards compatibility if others are using it.

You are also now a co-maintainer of Test::Output. I didn't expect you
to do the work. :)

Thanks for all the comments on the book, which I just handed over to
O'Reilly. I'd say more but it's way too early in the morning.
