Refactor apache module detection to be more realistic and fastcgi freindly #234

The code in apache_mod_loaded and got_mod_rewrite assumes that php is executed via the mod_php apache module.

We should make it more friendly in a setting which uses apache with fastcgi and at the very least assume that the modules which we require actually exists.
