Workshop Request Form - Membership Affiliation

On the new Workshop Request Form there is no way for a person to select which membership this workshop would be apart of.

There have been ideas to add a drop down menu that lists the Membership Organisations in AMY. Understand there are limitations to this approach. @fmichonneau and @tracykteal and @sheraaronhurt talked about this and for now we can reconcile manually, going fwd we can decide if box/drop down is the best solution and how things are being managed in AMY

My suggestion: we can pull the most actual, up-to-date membership based off of user selected Organisation (field "Institutional affiliation*" in the form). But I understand this issue doesn't say what solution is preferred?

This will be addressed once we implement the feature to enforce the member registration code.
