    The invariant I was using worked for pestc numbering, not for p4est
    numbering.  Thanks to Stefano Zampini for pointing both this and the
    previous bug out.

    created an additional permutation for face corners to avoid having to
    lookup corner indices where this is not necessary.
    Used for populating the entries of quad_to_half for hanging faces at
    tree boundaries

    custom numbering and default numbering differ on the orientation of
    child edges of a face.  It worked for custom numbering before, but not
    for default numbering.  It appears to work for both now.

    Introduce a counter that starts at 0 on forest creation.
    On every refine, coarsen, balance, partition call that changes the mesh,
    it increases by one.  It stays the same whin these calls are noops.
    The revision counter is not saved to disk.  On load it is set to zero.
