Example from a live site:

Table 'dragonweed.users' doesn't exist <-- this should be "dragonweed.chyrp_users"

UPDATE users SET is_approved = 1 WHERE email = 'XXXXXX@gmail.com'

Array
(
)

#0 /srv/www/dw-site/includes/class/SQL.php(222): Query->__construct(Object(SQL), 'UPDATE users SE...', Array, false)
#1 /srv/www/dw-site/includes/controller/Main.php(562): SQL->query('UPDATE users SE...')
#2 [internal function]: MainController->validate()
#3 /srv/www/dw-site/includes/class/Route.php(115): call_user_func_array(Array, Array)
#4 /srv/www/dw-site/index.php(22): Route->init()
#5 {main}
