Modernize classic plugin menu #324

For each row a plugin developer allocates in the classic plugin menu, a table with a single row is emmitted. That doesn't make sense at least nowadays, where a simpler structure appears to be more appropriate. Furthermore, the default styling of the plugin menu should be improved.

Close cmsimple-xh#324: Modernize classic plugin menu
As a first step we replace the single-row tables with `<ul>s`.
