Failed /meta/connect retried with existing /meta/connect #800

When the client has an outstanding /meta/connect request, it may issue a new /meta/connect request - for example because of reloads or because of timeouts.

In such case the old /meta/connect should be failed and no further action taken.

Instead, the failure of the existing /meta/connect causes a retry, and now there are 2 outstanding /meta/connect requests; the second will cancel the first; the failure of the first will cause a third retry, which cancels the second; and so on.

This affects both the Java client and the JavaScript client.
