Refactor loader Qdrant helpers into dedicated module

Summary

• move the Qdrant helper constants and functions into mcp_plex/loader/qdrant.py and re-export them from the loader package
• update loader wiring to reference the extracted helpers, bumping the project version to 2.0.3
• refresh loader unit tests so they import from the new module and continue covering collection/index setup and payload generation
