Using scrbook instead of book #34

I would suggest using the document class scrbook.
This is part of KOMAScript
(http://www.ctan.org/tex-archive/macros/latex/contrib/koma-script/scrguien.pdf/)
and uses some reasonable default settings for documents. It uses A4 by
default, so the option is not needed anymore.

Does it use A4 paper on all locales?

I'm not sure if I understand your question correct. My commit uses A4 by default. You can add a class option to use another format:
\documentclass[a5paper]{scrbook} --> uses A5
\documentclass[letterpaper]{scrbook}-> uses US Letter

But paper format is independent from the system locale. So if some american user uses en_US (or something equivalent) on their system, scrbook will use A4 format without any other option.
