Ability to set the 'app' that CementTestCase.make_app() uses #166

For example:

from cement.utils import test
from myapp.cli.main import MyTestApp

class MyTestCase(test.CementTestCase):
    app_class = MyTestApp

    def test_something(self):
        # this should be an instance of MyTestApp
        self.app.setup()

        # additionally, this should be as well
        app = self.make_app(config_defaults=dict(foo='bar'))

This can be backported to stable/2.0.x as it would not affect current functionality.
