Plugin extension does not disable after already being enabled #246

Based on the config load order, if a plugin is loaded and added to "enabled" plugins, but then a second config in the change loads a config for that same plugin that has it "disabled" the plugin is now both enabled and disabled. The plugin handler should remove the plugin from active/disabled (oposite of what it is adding it to).
