Such practices are definitely illegal, we recently reported on a successful complaint against something similar: https://www.datenanfragen.de/blog/abis-unterschrift-dsgvo-anfragen/
I'll verify this with a request myself and if they still do this, I'll submit a complaint.

Seems like someone else already complained about that. While they do still ask you to fill they form, they don't require that anymore:
Sollten Sie das Formular nicht ausfüllen wollen, werde ich für Sie am Freitag, den 15.10.2021, intern die Anfragen für eine einfache Auskunft stellen.

And as for the ID copy: They only require that if you want to get access to your bank details or credit score, which is reasonable enough:

I see that the record is already marked as "tested". For the sake of completeness, I would like to add that I have not yet complied with the request to send the form and a copy of the ID card by post.
