  3. switched to using db:migrate and db:seed directly

there is no need to allow katello to create and destory the pg db,
however rake db:setup drops and creates the db, while not an issue with sqlite...
    fixed a few ordering issues

  1. app server - updates to use thin Rack handler vs script/thin

Prior to this change, we found that developers using rubymine could
not really run the app within the IDE using the thin app server
and applying the /katello prefix. This change is to address that issue,
but it applies a change to how a developer will start katello in a dev
A few options that exist are:
Scenario 1 (recommended): Start using prefix and thin:
      RAILS_RELATIVE_URL_ROOT=/katello script/rails server thin
Scenario 2: Start using thin w/o prefix:
      script/rails server thin
    Scenario 3: Start using webrick:
      script/rails server
Note: for scenario 3, if user specifies "use_ssl: true" in the
katello.yml, the service will be accessed with https; otherwise,
      it is http.
For thin, SSL direct access is not supported by the above scenarios.
If SSL is required with thin config, it is recommended to use an Apache front-end
similar to the production deployment. The reason being the RAILS Rack
Handler for Thin doesn't currently provide mechanism to specify
the SSL details (e.g. enable, key, cert, verify...)

  2. script/rails - adding back in... needed to run rails console

As part of the thin migration we removed script/rails. The reason being
developers should start the application using script/thin instead. Today,
we found that "rails console" is no longer working. It turns out that in
order for that to work, script/rails must exist.
I am re-adding the original script/rails back in. This script includes
both the standard logic that is provided by "rails new katello" as well
as the custom code we had placed in it for handling SSL when using webrick
and looking at version info for things like candlepin.
It should be noted, that we still do not intend for developers to run
the app using script/rails (Please use script/thin), but if they need
to access console, "script/rails console" is available.

  3. renaming the 2 providers to something more useful

no reason to call the custom provider 'porkchop' anymore. has no
    meaning to our users.

  5. fake-systems - removing scripts, use instead

The fake systems scripts are being removed. With the transition to using thin,
these scripts no longer work; however, an alternative script that should be used
is The bulk load script not only creates dummy systems,
but also organizations, providers, products and repos.
