Add customizer.js (https://gist.github.com/mfields/9140890) to the extensions directory.
Add support via options-functions.php (https://gist.github.com/mfields/9140990).

I've only added basic support for core features and one custom setting. It should be pretty easy to add support for other custom settings as well.
