2.4.2: Multiple Placeholders issue #2126

I have an issue that seems to occur with the following temlpate:

{% extends "base.html" %}

{% load cms_tags sekizai_tags %}
{% load staticfiles %}
{% load menu_tags %}
{% load placeholder_tags %}

{% block base_content %}

{% block breadcrumbs %} {% show_breadcrumb 0 "menu/breadcrumbs.html" 0 %} {% endblock %}
{% block banner %}
    <div class="colspan12 bannerwrapper">
        {% placeholder banner %}
    </div>
{% endblock %}

{% block leftcol %}
    <div class="colspan9 panel plain">
        <div class="imageheader">
            {% placeholder sub_banner %}
        </div>

        {% placeholder left_column %}
    </div>
{% endblock %}

{% block rightcol %}
    <div class="colspan3 page-right-col ">
        {% placeholder right_column %}
    </div>
{% endblock %}

{% endblock %}

On attempting to publish an existing page that has been updated an error is being thrown that there are multiple placeholder with the same slot name, specifically this only occurs for 'sub_banner'. Is this a known issue with 2.4.2? I am using MySQL InnoDB, Python 2.7.3, Django 1.5.1 on CentOS 5.9 (I know, legacy system - I have no choice >.< ). Any help would be hugely appreciated on this

Does this happens on newer version of 2.4 branch?

Cannot reproduce.
What's the content of "base.html"?
