The documentation for generate.index suggests that it can be used
to forbid autogeneration of an index, but that was not true. Now
we perform a check to make sure that, even if the index element is
empty, generate.toc is not set to false.

Fixing an issue where the default value for olink.doctitle is an integer
but the stylesheets expect it (in one place) to be a string.

Glossary rendering and preprocessing had some issues when more than
one `glossterm` element was present inside a `glossentry`.
