takitapart's repo is now just a fork of the plugin on docpad; apparently, the multimarkdown plugin was adopted by docpad and so docpad should be the primary repo that is linked

- Updated Azure link as the tool name has changed to xplat. (The old link was redirecting.) - Pointed broken Windows Azure deployment link to the updated URL - Removed Nodejitsu section as Nodejitsu has been bought by Godaddy, and is no more. - The linked Modulus.io help page was broken and redirected to a generic support search page. Therefore, I pointed it to the most sensible replacement for the original link. - Changed docker deployment link, as the RobLoach/Forge link is gone now.
