Custom question fields

The idea here is that administrators can add questions for users, projects, and even activities. Basically a form builder for integrated surveys. I'm not yet sure of the direction or design here.

Closing for now, because I don't have the time or the incentive to finish this. Instead, I would recommend integrating a cloud form product, or if you want to go the full on open source way, look into:
