A specific scenario, where rolling restart of a single node, without any metadata change, can cause loss of metadata. For example, starting one, working against it, then starting another node, shutting down the first node, shutting down the second node (without any metadata changes, like new indices / updated mappings), and starting up the second node, can cause metadata loss.
